At Summit’s Trace Healthcare Center we understand that from infancy to childhood;

We are a Medicare/Medicaid certified center offering services ranging from comprehensive
rehabilitation to assisted living, 24-skilled nursing and Alzheimer’s care. Our philosophy

the level they require in order to preserve their quality of life, self sufficiency and
independence at the highest possible levels. As a continuum of care community, we offer
options for stepped-up or stepped-down care that addresses the specific needs of each
